Job description
Conduct initial psychosocial assessments to identify socioeconomic influences on patient treatment strategies. Work alongside the interdisciplinary team to create care plans and ensure precise clinical documentation is maintained. A valid Licensed Clinical Social Worker (LCSW) credential in California is required for this position. Additionally, candidates should have a current driver’s license, dependable transportation, and up-to-date First Aid and CPR certification.
