Job description
The MDS Coordinator oversees the Resident Assessment Instrument (RAI) process and ensures the completion of precise Minimum Data Set (MDS) evaluations. This role includes active participation in interdisciplinary team meetings to create thorough care plans for residents. Applicants should possess a current, unrestricted LPN license and be well-versed in state and federal regulatory standards. A minimum of one year of direct nursing experience along with MDS/RAP knowledge is preferred.
