**Job Title: Transplant Coordinator**
The transplant coordinator is responsible for conducting thorough patient evaluations and formulating collaborative care plans aimed at achieving the best possible outcomes for patients. This role includes delivering patient education, facilitating communication among health team members, and ensuring that all documentation meets established clinical standards. Applicants are required to possess an Associate’s degree in nursing and hold a current RN license in Texas. Additionally, a minimum of three years of experience in Medical/Surgical or Intensive Care environments is necessary.
