Job description
**Job Title:** Assertive Outreach Specialist
In this role, you will engage in proactive outreach, provide support, and implement therapeutic interventions for clients within community environments, aimed at enhancing their quality of life and treatment results. You will be responsible for crafting and overseeing individualized treatment plans, as well as coordinating necessary crisis management and emergency services. A bachelor’s degree in a human services discipline or a comparable blend of education and experience is required. Applicants should demonstrate excellent communication abilities, hold a valid driver’s license, and successfully complete background and drug screenings.
