Job description
**Job Title: Case Manager**
The Case Manager plays a crucial role in supporting individuals with intellectual disabilities and their families by assessing needs, creating care plans, and facilitating service referrals. This position involves coordinating various services, advocating for the preferences of the individual, and overseeing their health, safety, and progress as part of a multi-disciplinary team. Candidates should possess a bachelor’s degree in human services or a similar field, or a degree in an unrelated discipline along with a minimum of one year of experience in adult intellectual and developmental disabilities (IDD) services. A valid driver’s license, a dependable vehicle with proper insurance, and proficiency in English are mandatory qualifications.
