Job Summary
The Advanced Practice Registered Nurse (APRN) takes on the essential task of visiting various facilities to carry out patient evaluations, establish diagnoses, and create thorough care strategies. They work closely with interdisciplinary groups and physicians to uphold superior standards of post-acute care while ensuring precise clinical documentation.
Responsibilities
- Perform patient assessments and diagnose medical conditions.
- Develop comprehensive care plans tailored to individual patient needs.
- Collaborate with interdisciplinary teams and physicians for effective patient management.
- Maintain accurate clinical documentation to track patient progress and treatment outcomes.
- Conduct physical examinations and order necessary diagnostic tests.
- Prescribe appropriate treatments based on patient evaluations.
- Educate patients about disease management and preventive care strategies.
- Participate in weekend call rotations as required.
Qualifications
- Must hold a valid Nurse Practitioner license.
- Experience in conducting physical examinations and managing patient care is essential.
- Ability to work collaboratively with healthcare teams.
- Strong communication skills for effective patient education and interaction.
- Willingness to engage in evening and weekend on-call duties.
