The RN Case Manager is responsible for overseeing the development of empathetic, patient-focused care strategies for individuals and their families. This role will involve independent work in the community while fostering significant relationships to ensure patients receive comprehensive support. Applicants are required to possess a valid Colorado Nursing License and be bilingual in both English and Spanish. A minimum of one year in home health or hospice settings is preferred, accompanied by excellent organizational and communication abilities.
