Job Summary
The Care Coach is responsible for guiding individuals in their health and wellness journeys by providing personalized support and encouragement. This role emphasizes proactive engagement with clients to foster positive behavioral changes, resulting in improved health outcomes.
Responsibilities
- Engage with clients regularly to monitor their progress and offer support.
- Develop tailored health plans that align with individual client goals.
- Provide education on wellness topics, including nutrition and physical activity.
- Assist clients in overcoming barriers to achieving their health objectives.
- Document client interactions and progress to ensure continuity of care.
- Collaborate with healthcare professionals to create a comprehensive care approach.
Qualifications
- Proven experience in coaching, counseling, or a related field.
- Strong understanding of health and wellness practices.
- Excellent communication and interpersonal skills.
- A background in healthcare or social services is preferred.
