Job description

Care Coach I 
Tampa, FL (remote) 
$30.50/HR Care Coach I plays a vital role in supporting members on their journey toward improved health and wellness. This position assesses and evaluates member needs, identifies barriers to care, and guides members and their families toward appropriate resources to help achieve and maintain optimal health outcomes. The role requires frequent interaction with members, families, healthcare providers, and community partners. This is a patient-facing position that involves local travel to healthcare facilities and community settings. Care Coach I Responsibilities:

  • Assess and evaluate member health needs, goals, and challenges
  • Guide members and families in accessing appropriate healthcare, social, and community resources
  • Employ coaching strategies to support behavior change and health improvement
  • Identify and help resolve barriers to effective care
  • Monitor member progress toward established health goals through ongoing assessment, data review, and member conversations
  • Maintain accurate and timely documentation across multiple electronic systems
  • Actively participate in care planning and follow established procedures, regulations, and professional standards
  • Communicate effectively with members, families, providers, and internal teams
  • Manage assigned workload independently while following documented guidelines and direction

Care Coach I Requirements:

  • Bachelor’s degree
  • 2+ years of related professional experience
  • Bilingual in Spanish and English
  • Strong working knowledge of Microsoft Word and Excel
  • Ability to use multiple electronic applications and documentation systems
  • Exceptional verbal, written, and interpersonal communication skills
  • Passion for improving consumer and patient experiences
  • Valid state driver’s license
  • Proof of personal vehicle liability insurance with minimum limits of 100,000/300,000/100,000
  • Ability to meet requirements of Humana’s Driver Safety Program
  • Ability to complete Tuberculosis (TB) screening, as this is a patient-facing role

Care Coach I Preferred Requirements:

  • Master’s degree
  • Experience working with Medicare and Medicaid populations
  • Previous experience with electronic case note documentation across multiple systems
  • Background in health promotion, coaching, or wellness
  • Knowledge of community health organizations, social service agencies, and local community resources
