**Job Title: Care Coordinator**
The Care Coordinator will evaluate, develop, and oversee the support services for youth experiencing significant emotional challenges, employing a strengths-focused methodology. Key responsibilities encompass managing a case load, leading Child and Family Team discussions, and collaborating with both internal and external care providers. Applicants are required to possess a Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a similar discipline. A minimum of 2 years of experience in clinical treatment or care coordination with children and adolescents is necessary.
