Job description
**Job Title: Care Coordinator**
The Care Coordinator is accountable for overseeing a caseload of individuals with significant needs, helping them access vital services such as housing, mental health care, and social assistance. This role includes providing crisis intervention, tracking client development, and coordinating with a diverse team of professionals to facilitate smooth transitions in care. Applicants should possess at least a High School Diploma or GED, with a preference for candidates holding an Associate’s degree in Human Services or Social Work. Additionally, candidates must have a minimum of 1-3 years of experience in case management and a valid driver’s license from New York State.
