Job Summary
This role is focused on overseeing the discharge and transition of patients, facilitating communication and coordination between various medical teams and families. The individual will assist doctors by supplying essential information to referral entities and verifying that all preparations are made for post-acute care.
Responsibilities
- Coordinate the patient discharge process with multidisciplinary teams and family members.
- Provide physicians with critical information needed for referral agencies.
- Ensure that all necessary arrangements are in place for post-acute care readiness.
Qualifications
- A high school diploma or GED is required.
- An associate’s or technical degree is preferred.
- Previous experience in a healthcare environment is advantageous for this position.
