The coordinator manages care activities for patients and families, including developing individualized care plans and facilitating communication between providers. They also perform outreach in various settings to ensure patients remain connected to primary care and follow-up services. Candidates must have a high school diploma or equivalent and 1-3 years of relevant experience. A bachelor’s or master’s degree in a relevant field and experience in health home or care management are strongly preferred.
