The MSW Care Manager delivers holistic, patient-focused care management for at-risk individuals in an acute care environment. Key responsibilities encompass assessments, discharge planning, coordinating care, and recognizing resources to help overcome social or financial obstacles to treatment. Applicants are required to possess a Master’s degree in Social Work from a CSWE-accredited institution. A minimum of one year of social work experience in a healthcare context, along with experience in acute care or a similar orientation program, is mandatory.
