**Case Manager RN**
The Case Manager RN is responsible for promoting communication and coordinating care throughout the continuum of patient services. This role involves working closely with patients, their families, and a diverse team of healthcare professionals to guarantee efficient transitional care management. Applicants are required to possess an active Registered Nurse License in California and a minimum of 2 years of experience in a healthcare environment. A Bachelor’s degree in Nursing is desirable, and candidates should either have 2 years of experience in Case Management or have successfully completed the Transitions in Practice program.
