Job Summary
The Transition Coordinator is responsible for delivering personalized instruction and support to surgical patients while acting as their advocate during the recovery phase. This role emphasizes effective discharge preparation, coordination of ongoing care, and maintaining seamless communication between patients, surgeons, and primary care providers.
Responsibilities
- Provide individualized education and instruction to surgical patients.
- Act as an advocate for patients throughout their recovery journey.
- Oversee discharge planning to ensure patients are well-prepared for post-surgical care.
- Coordinate follow-up care to enhance patient outcomes.
- Facilitate communication between patients, surgeons, and primary care physicians to ensure continuity of care.
Qualifications
- Bachelor’s Degree in Nursing is required.
- Current Registered Nurse License in Washington state.
- Previous acute care experience in the relevant specialty area is necessary.
