**Job Title: Case Manager**
The Case Manager plays a crucial role in offering assistance and guidance to individuals facing homelessness, striving to facilitate their journey towards self-sufficiency and stable housing. Responsibilities include performing assessments, crafting personalized case plans, and advocating for the needs of clients while ensuring thorough documentation within the HMIS system. Candidates should hold an associate degree in a social or behavioral sciences discipline and have a minimum of one year of experience in the social services sector. Additionally, applicants must have a valid driver’s license, successfully pass a background check for fingerprint clearance, and achieve certifications in CPR and crisis de-escalation.
