Job Summary
The Case Manager plays a pivotal role in coordinating and facilitating care for clients, ensuring that they receive the necessary support and services tailored to their individual needs. This position emphasizes holistic approaches to case management, considering physical, emotional, and social factors affecting clients’ well-being.
Responsibilities
- Assess clients’ needs and develop personalized care plans.
- Collaborate with multidisciplinary teams to deliver comprehensive services.
- Monitor client progress and adjust care plans as necessary.
- Advocate for clients to access community resources and support.
- Document case notes and maintain accurate records in compliance with agency standards.
Qualifications
- Minimum of a Bachelor’s degree in Social Work, Psychology, or related field.
- Experience in case management or a similar role is preferred.
- Strong communication and interpersonal skills.
- Knowledge of social services systems and community resources.
- Ability to work independently and as part of a team.
