Job Summary
The Case Manager plays a crucial role in facilitating and coordinating care for clients, ensuring that they receive the necessary support and resources to achieve their goals. This position involves establishing relationships with clients and working collaboratively with other providers to promote a holistic approach to care management.
Responsibilities
- Assess client needs and develop appropriate care plans.
- Coordinate services and resources tailored to individual client requirements.
- Monitor client progress and make necessary adjustments to care plans.
- Maintain accurate and timely documentation of case notes and client interactions.
- Advocate for clients to ensure access to services and support.
- Collaborate with multidisciplinary teams to enhance client outcomes.
Qualifications
- Degree in social work, psychology, or a related field.
- Experience in case management or related services.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of community resources and services.
- Relevant certifications are a plus.
