Job description
**Job Title: Case Manager**
The Case Manager plays a vital role in performing intake assessments, creating tailored service plans, and delivering continuous support and advocacy for clients. Responsibilities also include maintaining accurate administrative records, intervening in crisis situations, and collaborating with community organizations to coordinate services. Applicants should possess a Bachelor’s degree in behavioral sciences or have a minimum of two years of pertinent experience with homeless populations or individuals facing mental health and substance use challenges. A valid driver’s license is necessary for travel related to business activities.
