Job Summary
The Case Manager plays a crucial role in facilitating patient care by managing referrals, offering health education, and assisting patients in overcoming obstacles to access services. Qualifications
- Must hold a valid nursing license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Oklahoma.
- Strong communication skills to effectively interact with patients and healthcare teams.
- Excellent organizational abilities to manage multiple patient cases.
- Capability to work within an interdisciplinary team environment.
