Job description
The Case Manager will create and oversee personalized service plans, coordinating care for clients dealing with substance use disorders and concurrent conditions. This role also entails advocating for clients to guarantee their access to vital services while maintaining precise documentation in line with agency guidelines. Applicants should possess either a Certified or Registered Substance Use Disorder Counselor credential and have a minimum of two years of experience in the behavioral health field. Additionally, a valid driver’s license from California and successful completion of thorough pre-employment screenings are mandatory.
