Job description
**Case Manager**
The Case Manager will be responsible for performing in-person evaluations of clients, creating customized care strategies, and executing quality assurance visits in the field. This role will also act as the main liaison for families and offer guidance to caregivers concerning client requirements. Applicants should possess a minimum of 2 years of experience in case management, intake, or social work specifically in home care or senior living environments. A strong understanding of senior care needs, assistance with activities of daily living (ADLs), and dementia care guidelines is essential, as well as access to dependable transportation for local travel.
