Job Summary
The role is focused on managing and streamlining patient care by working closely with the Interdisciplinary Care Transitions team. The objective is to ensure that resources are used efficiently, and that patients receive continuous and satisfactory care throughout their journey.
Responsibilities
- Facilitate collaboration among healthcare teams to enhance patient care delivery.
- Conduct thorough assessments and develop tailored care plans for patients.
- Manage discharge processes effectively to ensure smooth transitions for patients.
- Promote ongoing communication among all stakeholders involved in patient care.
- Monitor and evaluate patient satisfaction and care outcomes.
Qualifications
- A postsecondary degree from a recognized nursing institution.
- Possession of a valid RN or LCSW license.
- Minimum of 2 years’ experience in a healthcare environment, with a background in case management or utilization review preferred.
