The Case Manager acts as a bridge between residents, clinical teams, and managed care partners to ensure high-quality care and operational excellence. Responsibilities include managing authorizations, supporting PDPM classification, coordinating safe discharges, and ensuring strict regulatory compliance. Candidates must hold an active RN or LPN license in the State of Michigan. A minimum of 2 years of experience in case management, utilization review, or MDS within a healthcare setting is preferred.
