**Job Title: Centralized Follow-Up RN**
The Centralized Follow-Up RN is responsible for overseeing abnormal laboratory results, monitoring referrals, and guaranteeing prompt communication with patients about their care plans. This role includes managing the nurse line, providing educational support to patients, and working closely with health center personnel to ensure precise documentation and continuous patient care. Applicants must hold a Registered Nurse license in New York State and possess a minimum of one year of experience in clinical or hospital nursing. Proficiency in electronic health records is essential, and strong interpersonal skills are mandatory. Preference will be given to candidates with a background in women’s health or family planning.
