Job description
Job Summary
The Clinical Care Nurse plays a crucial role in delivering high-quality patient care by utilizing their clinical expertise and nursing knowledge. Through direct patient interactions, they provide comprehensive assessments, implement care plans, and collaborate with a multidisciplinary healthcare team to ensure optimal patient outcomes.
Responsibilities
- Conduct thorough patient evaluations and assessments to determine healthcare needs.
- Develop, implement, and adjust care plans based on individual patient requirements.
- Collaborate effectively with physicians and other healthcare professionals to ensure coordinated care.
- Educate patients and their families on health management and wellness strategies.
- Document all patient interactions and care provided accurately and promptly.
- Participate in continuous professional development and training programs.
Qualifications
- Registered Nurse (RN) license in the state of practice.
- Demonstrated experience in patient care settings, preferably in a clinical environment.
- Strong communication and interpersonal skills for effective collaboration.
- Ability to assess, plan, implement, and evaluate patient care.
- Current certifications in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) preferred.
