**Community Health Care Advocate**
The Community Health Care Advocate plays a vital role in connecting healthcare providers with the community by aiding in the execution of care plans and coordinating patient services. This position involves outreach efforts, health education initiatives, and facilitating connections to preventive health services to enhance patient outcomes. Applicants should have a High School Diploma or GED and must acquire a Community Health Worker (CHW) certification within a year of employment. Preference will be given to candidates with at least one year of experience in medical assisting, particularly in an OB outpatient setting.
