Job Summary
The Field Care Manager Nurse (RN) plays a vital role in overseeing and coordinating the care of patients in a community setting. This position requires a skilled registered nurse dedicated to ensuring quality patient care and enhancing health outcomes. The Field Care Manager will collaborate with various healthcare professionals to deliver comprehensive care tailored to individual patient needs.
Responsibilities
- Conduct assessments of patients to determine appropriate care plans.
- Coordinate care among healthcare providers to ensure a seamless patient experience.
- Educate patients and families on health management and resources available.
- Monitor progress and adjust care plans as necessary based on patient responses.
- Document patient interactions and maintain accurate records for compliance and continuity of care.
- Advocate for patients’ needs within the healthcare system.
Qualifications
- Must hold a valid RN license in the state of practice.
- Experience in case management or community nursing is preferred.
- Strong communication skills to work effectively with patients and healthcare teams.

- Proficient in using electronic health records and other medical software.
