**Job Title: Discharge Planner**
The Discharge Planner plays a key role in coordinating discharge appointments and setting treatment objectives for patients in a fast-paced inpatient psychiatric setting. This position involves working closely with a multidisciplinary team as well as community partners to facilitate seamless transition planning for patients. A bachelor’s degree in psychology or a related discipline is essential, along with a minimum of one year of experience in mental health services. Strong organizational and communication abilities are crucial, and preference will be given to applicants who have experience in crisis units.
