The Care Coordinator will focus on lowering homelessness rates among people facing substance use issues by delivering crisis intervention and stabilization assistance. Responsibilities include managing care plans, tracking treatment advancements, and connecting clients to services in community settings. Applicants should have a Bachelor’s degree in a Human Services discipline or an Associate’s degree accompanied by two years of direct care experience. A valid New York State driver’s license and dependable transportation are essential for this position.
