**Job Title: Family Nurse Practitioner**
The Family Nurse Practitioner delivers thorough primary and preventive healthcare services throughout the individual’s life, addressing both acute and chronic health issues while working collaboratively within a multidisciplinary team. This position entails educating patients, offering health coaching, and engaging in coordinated clinical care management to enhance health outcomes in the community. Applicants should possess a minimum of three years of clinical experience and a Master’s degree or higher from an accredited Nurse Practitioner program. Essential qualifications include an active APRN/NP license in California, DEA registration, and board certification in Family Practice.
