Job description
The RN Case Manager is responsible for overseeing nursing care and coordination for a designated hospice caseload in multiple residential environments. Key duties involve performing clinical assessments, managing symptoms, and working alongside an interdisciplinary team to develop and execute tailored care plans. Applicants must possess a valid Texas Registered Nurse license and have a minimum of one year of nursing experience, preferably in hospice care. This position demands excellent clinical assessment abilities, dependable transportation, and the capacity to independently manage a caseload.
