Job description
**Home Health Admission RN**
The Home Health Admission RN is responsible for performing initial assessments of patients, crafting tailored care plans, and ensuring the accurate completion of all documentation related to admissions. This role involves collaborating with the intake team, medical professionals, and various disciplines to address the needs of patients while adhering to productivity benchmarks. Applicants should have an active RN license in Wisconsin and a minimum of two years of clinical experience. Preference will be given to those with a BSN or two years of experience in home health settings, along with a valid driver’s license and strong communication abilities.
