Job Summary
The Home Health Admission RN will be responsible for performing initial patient assessments and developing comprehensive care plans for home health patients. This position plays a crucial role in ensuring quality care delivery and patient satisfaction within the home health care setting. The RN will collaborate with various healthcare professionals to ensure that patients receive optimal care according to their individual health needs.
Responsibilities
- Conduct thorough initial assessments of new patients to determine health care needs.
- Develop and implement personalized care plans in coordination with interdisciplinary teams.
- Evaluate and update patient care plans based on ongoing assessments and patient feedback.
- Educate patients and their families about care processes, equipment, and health management.
- Ensure compliance with healthcare regulations and standards during the admission process.
Qualifications
- Active and unrestricted Registered Nurse license.
- Experience in home health care, preferably with a focus on admissions.
- Exceptional assessment, analytical, and patient care skills.
- Strong communication and interpersonal abilities.
- Ability to work independently and manage time effectively.
