Job Summary
The Home Health Field RN is responsible for conducting thorough patient evaluations and creating tailored care plans to address intricate medical issues in a home environment. This role necessitates effective collaboration with multidisciplinary teams and involves delivering clinical education to both patients and their families to promote continuity of care and enhance health outcomes.
Responsibilities
- Conduct comprehensive assessments of patients to establish individual care strategies.
- Create and implement personalized care plans for managing complex health conditions.
- Work alongside interdisciplinary teams to coordinate care and streamline processes.
- Provide essential education to patients and their families to support health management.
- Ensure effective communication and follow-up to maintain quality of care.
Qualifications
- Current Registered Nurse (RN) license is mandatory.
- Valid driver’s license required for travel to patient homes.
- CPR certification is a must.
- Preferred experience of at least 2 years in home health care settings.
- Strong skills in clinical documentation are essential.
