Job Summary
The Home Health Field RN is responsible for delivering thorough nursing care, conducting assessments, and developing treatment plans for patients in their home environment. This role involves working closely with various interdisciplinary teams to effectively manage intricate medical conditions and maintain seamless continuity of care for patients.
Responsibilities
- Provide comprehensive nursing assessments and care for homebound patients.
- Collaborate with a multidisciplinary team to create and implement patient-specific treatment plans.
- Monitor and document patient progress and changes in medical conditions.
- Educate patients and families on health management and medication administration.
- Ensure the delivery of high-quality healthcare services in the home setting.
Qualifications
- Active and valid RN license.
- Holder of a current driver’s license.
- Certification in CPR.
- Preferred: Minimum of 2 years of experience in home health nursing.
