Job description
**Home Health Nurse**
The Home Health Nurse delivers direct patient care through conducting physical evaluations, administering medications, and implementing nursing interventions in both home and hospice environments. This role involves working closely with the healthcare team to create personalized care plans aimed at achieving safe and effective patient outcomes. Applicants must possess an active Registered Nurse license in California, a valid driver’s license, and current Basic Life Support (BLS) certification. A preference will be given to candidates with one to two years of experience in home health, hospice, or public health nursing.
