Job description

Job Summary

The Home Health Nurse I (RN) is responsible for delivering high-quality nursing care to patients in their homes. This role focuses on ensuring that patients receive the appropriate health services and support to manage their conditions effectively.

Responsibilities

  • Assess patients’ health status and needs in their home environment.
  • Develop customized care plans in collaboration with patients, families, and healthcare teams.
  • Provide direct nursing care, including administering medications and treatments as prescribed.
  • Educate patients and caregivers about managing health conditions and following care plans.
  • Coordinate with other healthcare professionals to ensure comprehensive care delivery.
  • Document patient information and progress accurately and timely.
  • Monitor patient outcomes and adjust care plans as necessary.

Qualifications

  • Current Registered Nurse (RN) license in the state of practice.
  • Experience in home health care or community health nursing preferred.
  • Strong clinical skills and knowledge of nursing best practices.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and manage time effectively.
