Days (M-F 8am to 5pm)
Flexible contract length-13 to 26 weeks
This is a full-time position (40 hrs/week ,traveling to patient homes in the Cinci rea. M – F, no weekends required, but flexible scheduling allows weekend visits if clinician chooses. Supplies for necessary therapy interventions will be provided (tablet and cell phone).
HH PT Responsibilities:
- The weekly productivity expectation is 7 visits/day or 35 visits per week.
- Geographic location of the position covers Downtown Cincinnati, Indian Hill, Colerain Twp., Norwood, Forest Park, and Sayler Park.
- Patient Population is adults and geriatrics.
- All diagnoses possible including COPD, PNA, CHF, orthopedic elective and trauma, Neuro, general med surg.
- Documentation is completed on the provided tablet using EMR Home Care Home Base. Company phone provided.
HH PT Qualifications:
- 1 year PT experience required
- State licensure required
- CPR certification required
- Home Care Home Base (EMR system) preferred
