Job Summary
The Physical Therapist is responsible for delivering tailored rehabilitative services to patients within their residences. This role involves evaluating patients’ functional abilities and formulating individualized treatment plans to enhance their recovery. The Physical Therapist works alongside a multidisciplinary team and oversees the support staff, while also providing education to patients and their caregivers regarding therapy objectives.
Responsibilities
- Conduct assessments of patients’ functional status.
- Create and implement personalized treatment plans.
- Collaborate with various interdisciplinary teams.
- Supervise and support staff in therapy operations.
- Educate patients and caregivers on therapy goals and practices.
Qualifications
- Possession of a current and active Physical Therapist license in California.
- Minimum of two years of relevant experience in the field.
- Current CPR certification.
- A valid driver’s license and access to an insured vehicle are necessary.
