**Job Title: Physical Therapist**
Provide expert, patient-focused physical therapy in a home health environment, working closely with a multidisciplinary team. Conduct thorough assessments of patients and create tailored therapy programs aimed at enhancing strength, balance, and overall independence. An active Physical Therapist license and a minimum of one year of clinical practice are required. Applicants should have a valid driver’s license and be willing to travel to the homes of patients.
