Job Summary
The Physical Therapist will evaluate the functional capabilities of patients and create customized treatment plans aimed at enhancing recovery in a home health environment.
Responsibilities
- Conduct assessments of patients to determine their functional abilities.
- Develop and implement personalized treatment plans tailored to each patient’s needs.
- Coordinate care and communicate effectively with physicians regarding patient progress.
- Provide education and support to caregivers for facilitating patient recovery.
- Supervise and support Physical Therapy Assistants in the delivery of care.
Qualifications
- Bachelor’s or master’s degree in physical therapy.
- Active California Physical Therapy license.
- A minimum of two years of relevant professional experience.
- A valid driver’s license and reliable transportation.
