Job description
The RN Case Manager is tasked with creating and modifying patient care plans in collaboration with a diverse team that includes LPNs and physicians. Responsibilities include performing home visits, conducting OASIS assessments, and offering educational support to patients and their families. Applicants must have a current RN license in Oklahoma and valid CPR certification. While experience in home health is desirable, candidates with robust assessment skills from a range of clinical backgrounds are also encouraged to apply.
