Job description
The RN Case Manager will be tasked with creating and modifying care plans for patients while collaborating with a diverse team that includes LPNs and physicians. This role involves performing home visits, conducting OASIS assessments, and offering educational support to patients and their families to promote favorable health results. Applicants must possess a valid RN license in Oklahoma and demonstrate strong assessment abilities along with critical thinking skills. CPR certification is mandatory, and although experience in home health is preferred, qualified nurses from various clinical areas are welcome to apply.
