Job description
The Registered Nurse Case Manager will create and modify patient care plans in collaboration with a diverse team of healthcare professionals, which includes Licensed Practical Nurses and physicians. Responsibilities include performing home visits, carrying out OASIS assessments, and educating patients and their families. Applicants must have a valid RN license in Oklahoma and demonstrate excellent assessment and critical thinking abilities. A current CPR certification is mandatory, and prior experience in home health is highly desirable.
