Job description
The Home Health RN Case Manager is responsible for overseeing and coordinating patient care through comprehensive assessments and the creation of personalized care plans. This role involves delivering skilled nursing services and supervising home health aides, all while adhering to regulatory guidelines. Applicants must possess a valid Registered Nurse (RN) license and have a minimum of two years of nursing experience. A current driver’s license and dependable transportation are necessary for conducting home visits.
