Provide expert, patient-focused nursing care within a home health environment while working alongside a multidisciplinary team. Conduct patient evaluations, oversee medication management, and instruct patients and their families on disease management strategies and treatment options. An active Registered Nurse license in the relevant state and a minimum of one year of clinical nursing experience are essential. Applicants must have a valid driver’s license and the capacity to travel to patients’ residences.
