**Hospice CNA**
The Hospice CNA is tasked with delivering personalized care to patients either at home or within healthcare facilities, adhering closely to individualized care plans. This role includes documenting patient observations, tracking vital signs, and alerting the clinical team to any important changes in the patient’s condition. Applicants must possess a valid CNA license in Pennsylvania and have a minimum of one year of relevant experience in nursing, home care, or hospice settings. Additionally, proficiency in Microsoft Office, familiarity with EMR systems, and a willingness to travel to various patient locations are essential.
