**Job Title: RN Case Manager**
The RN Case Manager delivers patient-focused, evidence-driven nursing care by collaborating with a multidisciplinary team for treatment and advocacy. Key responsibilities include evaluating patient conditions, creating care plans, and overseeing clinical outcomes, all while prioritizing compassionate and safe care. Applicants should possess a valid Wisconsin RN license and an Associate’s Degree in Nursing (ADN), although a Bachelor’s of Science in Nursing (BSN) is preferred. This position mandates Basic Life Support (BLS) certification and the capability to conduct physical activities, including lifting weights of up to 50 pounds.
