Job description
**RN Case Manager**
The RN Case Manager delivers expert nursing care to designated patients by creating, executing, and assessing customized care plans in partnership with an interdisciplinary team. This role includes overseeing support personnel, ensuring precise clinical documentation, and offering healthcare education to patients and their families. Applicants should have an Associate’s or Bachelor’s degree in nursing along with a valid RN license in California. A minimum of one year’s professional nursing experience is necessary, with a preference for candidates who have experience in hospice, oncology, or emergency services.
