Job description
Evaluate, advise, and guide patients with intricate medical needs by creating personalized treatment strategies. Support adaptation to disabilities, connect patients with resources, and promote behavior changes within their home and community settings. A Master’s degree in Social Work and current state licensure as an LCSW are mandatory. Applicants should have strong skills in planning, organization, written communication, and interpersonal communication.
