Job Summary
The primary role involves delivering behavioral health support, psychotherapy, and care coordination aimed at assisting older adults and their families. This position emphasizes conducting comprehensive psychosocial assessments and engaging with a multidisciplinary team to formulate personalized care strategies.
Responsibilities
- Provide therapeutic support and psychotherapy services to older adults and their families.
- Conduct thorough psychosocial assessments to evaluate clients’ needs.
- Collaborate with an interdisciplinary team to create individualized care plans.
- Facilitate care coordination among various healthcare providers and support services.
Qualifications
- Must possess an active Texas Licensed Clinical Social Worker (LCSW) license.
- 3-5 years of experience in geriatric behavioral health or senior care is required.
