Job Summary
The Licensed Practical Nurse (LPN) in Home Health in New Castle plays a vital role in delivering personalized healthcare services to patients in their residences. This position involves providing nursing care, support, and education to patients and their families, ensuring a safe and comfortable environment for the healing process. The LPN will collaborate with other healthcare professionals to promote the patient’s overall well-being.
Responsibilities
- Administer medications and treatments as prescribed by the healthcare provider.
- Monitor patients’ health status and report any changes to the supervising nurse or physician.
- Provide personal care and support with daily living activities.
- Educate patients and families regarding health management and care plans.
- Document patient progress and maintain accurate health records.
- Assist with rehabilitation activities and monitor patient progress.
Qualifications
- Must hold a valid and active Licensed Practical Nurse (LPN) license.
- Previous experience in home health care is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently as well as part of a healthcare team.
- Basic CPR and First Aid certification is preferred.
