Job Summary
The role involves providing individualized skilled nursing care to a patient in their home environment, focusing on facilitating daily living activities, enhancing comfort, and promoting overall health and well-being.
Responsibilities
- Deliver one-on-one skilled nursing care in the patient’s residence.
- Assist with daily living activities and ensure the patient’s comfort.
- Monitor and document the patient’s condition and their responses to various care treatments.
- Administer prescribed medications and treatments as per care plans.
- Adhere to the established care plan and promptly report any significant changes to a supervisor or physician.
Qualifications
- Candidates must possess a valid RN or LPN license issued by the state.
- Applicants should have a valid driver’s license and a current auto insurance card.
- Must provide a Social Security card and a valid CPR certification.
- Need to submit results of a qualifying TB screening, which can include a two-step test, a recent chest X-ray (within the past 12 months), or QuantiFERON Gold results from the past five years.
