Job Summary
The Licensed Practical Nurse will undertake home visits to deliver skilled nursing services. This role encompasses administering medications, managing wound care, and conducting assessments of patients. The LPN is essential in monitoring the well-being of clients, accurately documenting care in electronic medical records, and collaborating with the clinical management team to ensure comprehensive care.
Responsibilities
- Conduct home visits to provide skilled nursing care to patients.
- Administer medications and perform wound care as needed.
- Assess and monitor patient conditions regularly.
- Document all care provided in electronic medical records promptly.
- Coordinate with the clinical management team to optimize patient care.
Qualifications
- Current LPN license in Pennsylvania.
- A minimum of one year of recent, verifiable clinical experience.
- Graduation from an accredited nursing program is required.
- Prior experience in home care settings is highly preferred.
